- Proof of Residency
- One of the following: Lease, real estate tax, or recent rent receipt
- Two of the following: Utility bill, vehicle or voter registration, library card or other mail with first and last name
- Verification of Income for each Guardian in the Household
- 2 consecutive paystubs for the past 30 days, W-2 form, most current tax return, proof of SNAP, SSI, TANF, or child support
- County Clerk Birth Certificate
- Photo ID
- Medical Card (if applicable)
- Up-to-date Physical
- Up-to-date Immunizations
- Dental Exam
- Lead Test With Results
- TB Test with Results
- Hemoglobin Test
- Allergy Medical Statement if Applicable
- Asthma Medical Statement if Applicable
- Medication Authorization Form if Applicable
- Proof of Residency
